A Study of Drug Utilization Review on Aminoglycoside Antibiotics




Abstract
A retrospective drug utilization review(DUR) study was conducted on commonly used aminoglycoside antibiotics(gentamicin, tobramycin, and amikacin) by reviewing patients charts. The audit criteria for the use of these antibiotics established by Henricks J.N. was selected, reviewed and utilized in DUR process. Total 218 charts of patients who treated with aminoglycosides in department of internal medicine from May 1 to July 30, 1987 were retrospectively reviewed. As the results, 182 cases(83.5%) were met with the criteria for the justification of use, while remaining 36 cases(16.5%) were deviated from the criteria. In analysing process indicators, several criteria including body temperature monitoring, maintenance dosage and WBC monitoring were relatively well documented showing the accepted levels above 90%, while loading dose administration, serum drug level monitoring, dosage adjustment to avoid toxicity and other measurements for monitoring toxicity were infrequently documented showing low accepted levels below 30% of cases. In outcome analysis, blood cultures after discontinuing drug were rarely documented showing low accepted level of 24$. Overall, general measurements for drug indication and monitoring clinical responses were frequently performed, while other measurements for maintaining adequate serum drug levels to prevent toxicity and maximize clinical efficacy and detecting toxicity were infrequently performed.
